What can you do in your Patient Portal?
Change your password
Communicate with our staff regarding your visit, glasses/contact lens prescription, records request, prescription refills, etc.
Ask Dr. Luscavage a question
View/Print/Download your visit note
After logging in, select Visit Info Tab in top left purple bar
Click on visit date (it will be blue)
Select Print/Download Icon
Add, edit or delete changes in your:
Contact Info (limited changes only)
Pharmacy
Medications
Past Medical History
Ocular History
Family History
Read Patient Education Information
How to log in:
For personal computers, type https://eyetoeyeophth.ema.md into a Mozilla Firefox or Google Chrome browser and hit enter (do NOT type www in front). If you do not have Mozilla Firefox browser, you can search it on google.com and download it for free.
If you have an Apple computer, you can open Safari and type in eyetoeyeophth.ema.md.

Resetting your password:
- Log into your patient portal using the computer-generated password that we gave you
- Click on “RESET PASSWORD” found in the upper right corner
- Type in a personal password of your own that you can easily remember and click “Set Password”.Your new password must have at least 7 characters and one number.
Communicate with us
- Click on “Contact Us” in the purple bar at the top left
- Click on envelope icon for New Message
- Click in “to” box and select Eye to Eye Staff or Luscavage, Lonnie
- In Subject box, type a brief reason for the communication
- In Message box, type your question/communication
- Click Send
- Check for response by logging in and clicking on Contact Us
